It has been shown that penicillin G procaine administration without penicillin G benzatine reaches higher concentrations in large animals, which is required for a notable activity of penicillin against more common pathogens sensitive to it. For example, one dose of 9,000 U/kg IM. With a dose like this it is possible to quantify penicillin to levels of 0.1 according to FDA recommendation, plasmatic peak concentration was 0.58 (+/- 0.15) y 0.44 (+/- 0.02) micrograms/ml. At this doses concentrations of 0.1 micrograms/ml are reached for one week. If a higher plasmatic concentration is needed, it is not recommended to elevate dose, but rather reduce dosing interval to 8 or even each 6 hours. In horses a dose of 20,000 UI/kg b.w. is the maximum tolerable (administered at several sites) every 8 hours as short interval and every 12 hours as long interval. It is not recommended to elevate dose, because procaine radical is hardly biotransformed in horses, being 20 times more susceptible to toxicity than humans. In fact it has been reported that most of the “allergies” to penicillin are not that, but a incapacity to transform procaine given a deficiency in blood pseudocolinesterase in horses. In case of a procaine toxicity (tremors, excitation and even prostration with convulsions) use diazepam is recommended.

ESPENFORT® contains streptomycin to form the classic synergy. Penicillin procain distributes in a 1:1 (plasma-tissues) proportion. Only well designed formulations as ESPENFORT®, guarantee a 100% activity. In case of specially sever infections, ESPENFORT® can be used in combination with CEFANEL® injected at different sites, because penicillin and cefalosporin have synergic effect (Plumb D.C. Vet. Drug Handbook, Iowa State Univ. Press, pp 472, 1995).
